Grants, up to $5,000, are available to help families who need financial assistance to pay for their child’s health care services, treatments or equipment not covered, or not fully covered, by their commercial health insurance plan.  For eligibility guidelines or to apply go online to www.uhccf.org. Dear Public Service Director,

 

The UnitedHealthcare Children’s Foundation (UHCCF), a non-profit 501 ( c ) 3,  is seeking grant applications from families in need of financial assistance to help pay for their child’s health care treatments, services or equipment not covered, or not fully covered, by their commercial health insurance plan.

 

Qualifying families can receive up to $5,000 per grant to help pay for medical services and equipment such as physical, occupational and speech therapy, counseling services, surgeries, prescriptions, wheelchairs, orthotics, eyeglasses and hearing aids.

 

As a broadcaster, we know you are committed to informing your listeners about important health issues. We ask that you broadcast the attached 30-or-60-second public service announcement audio file, or use the scripts below for your announcers to voice. Should you wish to inform your listeners on your public affairs program, we would be happy to work with you to coordinate a local guest.

 

In this public service announcement campaign we are encouraging parents and legal guardians to apply for grants of up to $5,000 for child medical services and equipment by completing an online application at www.uhccf.org. To be eligible for grants, children must be 16 years of age or younger. Families must meet economic guidelines, reside in the United States and have a commercial health insurance plan. Applicants may apply online at any time; there is no deadline. The PSA can go into rotation now through the end of 2013.

 

In 2012, UHCCF awarded more than 1,300 grants worth $4.1 million to families across the United States for treatments associated with medical conditions such as cancer, spina bifida, muscular dystrophy, diabetes, hearing loss, autism, cystic fibrosis, Down syndrome, ADHD and cerebral palsy. The UnitedHealthcare Children’s Foundation is dedicated to helping even more families in 2013.
